Please arrive 5-10 minutes early for your appointment for registration and to fill in an information questionnaire.

When you come to your appointment – Remember to bring:

• Referral from you doctor (you are required to have a referral click here for more information)
• Any X-Ray’s, Pathology Results, Hearing Tests or other relevant documents
• Your Medicare Card, Veterans Affairs Card, Health Fund Card
• A list of current medications and any known allergies

To cancel an appointment:
Please phone the office during business hours. We ask that you cancel your appointment at least 24 hours in advance so that your appointment can be allocated to another patient awaiting treatment.

Appointment Fees:
Payment of your consultation fee is payable on the day by Cash, Cheque, EFTPOS or Credit Card.

Post-Op Appointments:
During the postoperative period, the cost of visiting the doctor is included in the operation fee. The period this applies to varies according to the surgery.

Referrals

The need for a Referral:
It is very important to have a referral from you doctor, as it tells Dr Wallwork what you have been sent for and may give extra important information such as your illnesses or medications. It is also a requirement from Medicare that you have a current referral from either a General Practitioner or Specialist for an appointment. This is so that you can obtain a Medicare rebate on your consultation fee.

Referral – valid durations:
A referral from your General Practitioner is valid for 12 Months from the date of referral.

A referral from a specialist if valid for 3 Months from the date of referral.
